Tiny moving worms

We commonly refer to these as "whiteworms", they are harmless but a little unsightly at worst, however they're a great source of protein for smaller fish. They generally come from overfeeding or excessive debris in the gravel. You CAN get rid of them with a Parasite clear medication, but IMO it's a waste of time and $$, since they have nutritional value. If you want a simple way to remove the majority of them, put a bunch of pellets in a piece of nylon and leave it at least overnight then the next day the nylon bag should be full of them.

For starters, what fish are in the tank now??

Ugh that's discusting, what are they exactly? And I have baby Convicts in there and a snail with raphael catfish
 
Not a real expert on them but I just know plecos will take care of them within one day and won't reappear since they will eat it.

I had this a long time ago didn't know what was going on but I just bought a Pleco to help me get rid of them and it worked. Fast forward he grew more than a foot and sold him lol

that's all you need. Leave them alone, baby convicts were exactly how I got rid of mine. Just let Nature take its course. They will grow faster, cut back to feeding every OTHER day, if u don't see a reduction in worms, feed every 2-3 days.

I had planaria in my tank a few weeks ago, only noticed a few though in midwater, nothing like your infestation. They were finer than hair but definitely little worms because they were wriggling like mad. The ones in your tank are like stubby little fat things, or are there different varieties of planaria? I did a good gravel vacuum, cut down on feeding and haven't seen them since.
 
I recall reading in one of my books that paradise fish are relentless hunters of those worms.
