Table Salt Ich

You need at least 2 teaspoons per gallon to kill it. Any less and you risk just making it salt resistant. I'd recommend going to 4 Teaspoons per gallon.

If you have hardy fish, you could do it in 1 shot. Or you can increase it gradually, like start at 1 tsp per gallon and a few hours later bring it up to 2, etc.

Note: If using table salt, make sure it does not contain 'anti-caking agent' or 'yellow prussiate (of soda)'. That can be toxic.
 
none, you need untreated sodium, not table salt, most people use aquarium salt by api, including myself, but ive heard kosher salt and other pure salts (not table salt, which is NaCl) that are untreated work well

Untreated sodium is an alkali metal that will react violently (i.e. Explode) if added to water in it's pure form.

Aquarium Salt, Rock Salt, Table Salt, Kosher Salt, Sea Salt, etc, etc, etc. Whatever you want to call it, salt is salt. There are different purities, grain size, and additives which can be present which cause the different names. It's all based on NaCl, Sodium Chloride.

The only risk with table salt, as I said previously, is that it may contain anti caking agents which can be toxic.

Should be good to go then. Some people claim iodine is bad as well, which is a common addative to table salt, but it's in such a small amount that the salt level would kill the fish long before the iodine did anything. Iodine can be a benefit in aquaria as well. There are iodine additives out there for aquarium use.

There is debate on whether the anti caking stuff is really toxic too. Some say yes, others say no. I can't say for sure either way. It's easy enough to find salt that doesn't have it though, so I figure it's probably not worth the risk.
