Doesn’t look like ich. Maybe some fungus or lymphocystis. Hard to tell with the pics. Increase your water changes to bi-weekly and add salt.
 
Oh dang, I was so sure it was ich. She’s in a 10 gallon hospital tank getting treated with ich-x, and I’m also treating my 135 gallon for ich… yesterday I saw my clown loaches rubbing their back on logs so I zoomed in for a pic - is this ich on them?

That looks like ich on the loaches. Didn’t look like that on the severum.
