It’s a response to some irritant. Bacteria, virus or stress. Would do a 50% water change now. Check your water parameters. Watch and see if it gets better. If still acting then consider seachem paragaurd.

I had Gymnothorax P. that suffered from this exact issue and unfortunately he passed, its usually an issue caused by infections of the skin through tears or other wounds and water quality issues that may singe the skin of GP.

given that these fish are scaleless, they are far more susceptible to external variables like water quality and infection.

I believe that the issue is not pathogenic as at the time it had not affected my other fish or GPs in the tank

The flaking of slimy coat will eventually domino into the sticking of fine particles onto their skin like sand or debris, this issue is magnified when their eyes get covered. i noticed that the immune system of the afflicted eel was heavily sundered and as a result was much more susceptible to other illnesses like cloudy eye.

as mentioned above, I was unable to save it. I used Kanamyxin sulfate, praziquental, and also maracyn 2, perhaps I was too late in my actions but these medicines should have a positive effect on your eels if administered at early stages.

I would advise against magnesium sulfate baths for the eels in any capacity as I fear a possible overexpression of osmoregulation through the salts as scaleless fish have basically no external protectors if their slimecoat is burning off.

What worked? Did you figure out what the cause was or was it just a water change that fixed it?
I did a 75% water change on my 400 gallon, monitored them and woke up the next morning and it was literally all gone off them and they were zwimming and eating normal. Crazyiest thing ive seen. Brink of death to normal in 12 hours
