What is wrong with the face structure of my guppy?

Please see the pictures, this guppy over time the one side of the face structure got totally distorted.

is this a disease? If yes then why all my other fishes are fine and only this fish has this problem.

appreciate your advise. I am willing to put it down if it will cause any issue of my other fishes.

Perhaps a genetic defect but there is a few diseases that cause that, that I can’t put the finger on the name
 
Did this develop over time or did he always look like that?
It could be some sort of defect, but if it wasn’t always like that it could be due to bacterial infection. You can try treating with kanaplex.
 
When I got this guppy, I never paid attention so perhaps it had it from before. But overtime it became bigger and visible. I also see it as a defect because other guppies and other fishes don’t have any issue. Also this guppy is male and there are two females in the tank, this guy cannot knock off those female guppies.

Could be a result from inbreeding or not culling
 
Once a month water change is too low, make it weekly. I can't see how your numbers could be so low with that schedule. The growths could be a chronic reaction to the water, and some fish are more sensitive. Make sure your test kit hasn't expired (they can give bad readings).

That also reminds me of goldfish with overdeveloped wen that grows over their eyes.

Most fancy goldfish get wen overdeveloped wen over their eyes no matter water quality
I do agree the bad water could have caused this
 
I had a similar experience with some black skirt tetras. As they aged several of them got tumors like that. I chalked it up to bad breeding habits.
