What have I got going on here? Treatments?

Looks like septicemia. Can try some maracyn2. Make sure your tank is cycled and nitrates are less than 20ppm.

Tank is well established. It’s a 180 gallon with he and a pleco. Filter is an FX6 and 2 AC 110’s. I change water when nitrates hit 20.

I would add salt at 1tbsp/gal as well to break the bacterial regen cycle, and keep at that concentration with your water changes to decrease bacterial load in water column. Also stimulates slime coat regeneration which will help soothe the skin as the antibiotic deals with the pathogen.
