Discus with hole in the head?

Posting this for my father.

This looks like hole in the head but have never seen it in discus - seen it Oscar’s and pikes mainly.

What are thoughts - what is it and what’s the treatment?

I’d call that hith. Is this a wc discus? Post your water parameters. Would move to a qt tank. Would add salt 1 tsp per g. Add some metronidazole to the tank and feed nls hex shield. Would dip that fish in some methylene blue daily. Remove any carbon from the tank. Hope it gets better.
 
Yes, that is exactly what it looks like- and severe.
I would treat in quarantine with metronidazole and kanamycin.
Raise temperature to 88-90 degrees.
Treat for 10 days.

How is the water quality is this tank:ammonia, nitrites, nitrates?

Just an educated guess; IMO, it looks like a very advanced case of HIH, aka Hex. Judging by the glazed over eyes and body lesions, a bacterial infection has taken hold & it looks advanced too. I've never experianced HIH disease in my tanks. I've read where other fishkeepers used "Metronidazole" to treat it. The bacterial aspect might require addition meds as well. I believe the most mentioned antibiotics are Kanaplex & Furan 2. I don't believe the prospects look too good for that poor guy. Hope he pulls through.

Would keep temps 78-80 in this case.
