Echinodorus tenellus. It really enjoys light (see below), but you can cultivate it easily with multiple regular fluorescents in a tank of less height. Mine is 2.5' tall, that requires some lumens :D I used to keep that species years ago in a 20'' tank with 144 Watts of Sera fluorescent bulbs. Worked :thumbsup:

No CO2 - I'll post a thread in the planted forum about that.
lighting: 10 hours with all lights (specs see below) + 3 hours on half power. Besides, my tank goes 2 hours dark in the middle to inhibit algae growth.
Specs on lights: Coralife Freshwater Aqualight, Double strips, i.e. 4x 96 Watts Power Compacts.

150 lbs of 3M Colorquartz mix
The left back of the tank contains 45 lbs Sera Floredepot.
The foreground (even with the ground cover) only has Sera Florenette A.
I fertilize with Kent Freshwater Plant, Kent Botanica Vita, and I actually have to add nitrates (I use Kent Freshwater Pro Plant)

Plants:
Echinodorus tenellus (front, groundcover)
floating plants described above
Cryptocoryne Wendtii and C. crispatula are doing well.
Echinodorus rubin is the centerpiece, a smaller E. ozelot grows in the front
My favorites are several bulbs of Crinum natans. They grow real slow, but create beautiful, long leave like nothing else :D
Besides that, I have Anubias barteri var. barteri, but my A. barteri var. angustifolia isn't doing too well...
