Did you lower your nitrate levels?

It's probably a bacterial infection...

Nitrate is sitting at around 20 which should be fine given there are a number of predatory fish plus the ray in the tank. No other fish have shown any signs of issues.

Yeah, I said from the start that nitrate wasn't an issue. Valuable time was wasted chasing a red herring.

I'd look for a vet unless you have a fair amount of experience treating with antibiotics.
