Esha Hexamita is excellent treatment for HITH. Definitely up the water change frequency, nitrates are a little too high. I try and aim for 2 - 3 30% water changes per week and that keeps everything tip top and at zero for me. Good luck Im sure he'll be fine with lots of TLC.
