Its been 2 months my flowerhorn have not eaten and been pooping white.

What have i done to treat the infection:
- moved him to a small, 1 feet quarantine tank.
- Stopped feeding him altogether
- Metronidazole tablets( 200mg once a day) x 5 days with Daily water changes
- I did epsom salt treatment
- i also tried S2 for 3-4 days and did a water change

And now i have given up on this fish.
hes been sick for more than 2 months now, kok shunken, sunken belly, and overall body has become very small.

Here im posting a pic of him. T_T

Put him in a bigger tank with daily water changes in a room away from any traffic and see how he does. Keeping him in a smaller tank will stress him out which I think is one of many causes of the hump shrinking. kno4te kno4te

Id try the metronidazole in a syringe w/o needle and inject in through its mouth. I’d also suggest adding levamisole.

Sorry I have a couple of questions. How long did you have him before he stopped eating?
What are your parameters out of the tap?
You said you tried not feeding him, for how long and when did you begin trying again?
Did you try live or only pellets? Did you try using something like Garlic Guard to entice a feeding response?
Did you remove anything that might remove the meds from your water such as carbon, etc in your filter?
For the epsom salt treatment you did a bath? Did you do it at the same time as the Metro tabs?

As kno4te kno4te stated administering the meds internally maybe more effective as it sounds like an internal problem. As kno4te stated you can use or a syringe or I personally when I have no choice use a pipette. Though it can be tricky as you have to make sure the meds get past the gills down the throat so it can make it's way to the stomach. Something I imagine many would be nervous doing. If the fish is taking food but spitting it out you can try soaking the pellets in a epsom and Garlic Guard solution. You could also go one better as Kensfish has a metro pellet which you could soak in epsom and Garlic Guard.

I just got a fish a couple of weeks ago who was spitting and had the white poops. A Thai-Silk? I believe the stress of overseas shipping and then a second in a short time cross country ship stressed the little guy. As some FH's do he developed black markings and was spitting food and white pooping. I soaked Kens metro pellet as I described and even though he'd eat and spit in just under 2 weeks he started eating and pooping normally.

CZ Clear can help (especially developed for flowerhorns). My guess is that it contains metro in one pill and an antibiotic in the other (maybe oxytetracycline). It comes with a syringe. If this fish is 10"+ long, you will need 2 bottles, and to do the full treatment of all the pills. There's quite a few youtube videos on how to slowly insert the syringe and very slowly inject the medication into the mouth.

i've had him for 3 months before he developed this problem.
Im using cycled water from my main tank, all parameters are within range
He started having white poop since October. i stopped feeding him for 2-3 weeks and tried feeding again. when he refused to eat, i fasted him for another 2-3 weeks and tried again. now its been more than 2 months.
I have only been feeding him pallets but he caught the infection from another fish in the tank which was successfully cured with metronidazole.
I have never tried garlic guard, i will order it if u suggest it.
Currently, i only have a hang-on filter for the quarantine tank, no carbon, just a sponge.
The epsom salt bath, i did with and without Metronidazole tablets, on and off in the past 2 months.

Thank you for your response.
Im gna get garlic guard and try injecting the meds directly into his mouth

Hope all goes well.
