Could be a swim bladder injury. Traumatic and since it's so thin not due if it'll make it. See if you can get it upright with decor or plants and reduce the water. This will help. Use a qt tank if available. Clean water and pray it gets better.

I lowered the flow in the tank by a lot.. He's found a spot under some wood that allows him to kinda stay upright... Should I do a big water change?

So this occurred after getting stuck to the filter?
I have no idea.. Usually every morning I'll go see him and he'll come up to me, this morning I didn't see him and noticed he was just kinda stuck to the sponge... Suction shouldn't be very high but im assuming he just kinda floated around and eventally landed there

Agree with other posters. Isolate, daily WC, maybe even prophylactic meds. Powerfeed and hope for the best. I'm worried that the little fella doesn't have energy reserves to recover from this coz of how skinny he is.

I noticed my test vial I used to test ammonia changed to a blue after about 20 min so I retested and I have about .50 ammonia so I'm doing a large water change

Leave the water level as it's found a place to hide. Hope it gets better. Would adjust the water changes to your water parameters.

Water quality seems fine. Not sure if it was a bloodworm that started all this. If it eats then it's a good sign. Not sure about puffers....thought they will take other foods.

Ask Fat Homer Fat Homer for a little suggestion.

I tried kinda hand feeding him and he'll chace any worms that are moving but leaves them alone if their still... I got him to eat 1 though.. He still chases my hand and even started chasing 1 shrimp I have in there too but when we settles down he goes head down

Also just did about a 75% water change... I drained slowly and am filling as slow as I can to keep from knocking him around... He found a spot on the bottom where he can stay upright and also eat bloodworms that are still in there... I'll siphon out any extra food in the morning

I was thinking about posing paraguard but I'm unsure if it's safe for puffers
