My guess is ammonia burn. Your ammonia might be perfect now, but if your friend over fed it could have spiked (and gone by down by the time you tested).

As a general rule it's safer for fish to starve for a week than have someone else take care of them.
I always thought that ammonia burn would cause the fish to develop black splotches (which is actually the fish healing).
 
Looks very odd, maybe something has contaminated the tank?
It doesn't even look sore, almost like it's peeling or leustic....

Looks fungal to me. Also the fact that it keeps disappearing and coming back. Fungal infections can do this. I would try pimafix instead of mela.
Thanks for the idea, I switched from the melafix to a general cure, and I don't want to keep switching medications invade it'll have negative effects, so if the general cure doesn't do anything I try what you suggested
 
You know, now that I'm thinking of it, I did have an orange goldfish (looks just like a common, but both of his parents were white comet goldfish), that developed an white splotch on his head outside in the pond over the summer, that look exactly as being described here, like the orange was peeling off, due to a growth or something. And yes, a have seen goldfish turn from orange to white many, many times, and that was not it, it literally looked as if the orange was peeling off, exposing a white opaque growth underneath. I actually forgot about this incident, but when I checked on the fish, it was completely orange again, with no white growth on its head (for those unfamiliar, when goldfish turn from black to orange, or from orange to white, they almost never turn back).
 
think I would have gone with polyguard. general cure more of a parasite treatment. link below. but again I have gone with pristine water first.

mater of fact, kanaplex would be more straight to the point as i look back on things. good thing with kanaplex is it does not have MG, ( malachite green), with can be hard on your biofilter, not as bad as blue which should only be used in a dip/bath or hospital tank.
